Your prescription must not be more than 12 months old. |
Eyewear Request Form: |
To get an Eyewear Request Form select the link on the left and print. When you come to you appointment please make sure to have all appropriate approvals. |

Glasses: |
If you are an employee, base frames and lens are provided to you. Upgrades are available at an additional cost to the employee. |
