| Instructor's Comments | Equipment to Bring to Class: If qualifying on Extended Boom Lifts, bring Hard Hat, Self-Retracting Lifeline, and Full Body Harness to evaluation session; If qualifying on Vertical Lifts only, bring Hard Hat to evaluation session.
NOTE - Evaluations will be performed in the afternoon following the classroom session and, if needed, within a few days after the classroom session. Specific evaluation schedule will be established during classroom session. |
